You will guide the coordination of Care Home MDTs and facilitate weekly rounds with the GP practices ensuring that residents in need of a review or a discussion are identified and prioritised. You will provide expertise to help individuals prepare for, clinical discussions with primary care professionals. Working closely with Care Home Leads and other primary care professionals within the PCN, you will manage a caseload of care homes, ensuring timely assistance for staff and residents, and addressing changing needs. You will also support the implementation of local priorities, address health inequalities, and promote population health. This role is designed to improve communication between Primary Care and care home staff, aiming to prevent unnecessary GP visits and hospital admissions. The post holder must be adjustable, as the role will evolve alongside the Enhanced Health in Care Homes Framework initiative. Key responsibilities Proactively identify and collaborate with care homes to support personalised care for residents. Assist residents and families in managing their healthcare needs, facilitating referrals to the Care Home MDT or escalating issues to the Clinical Team. Promote shared decision-making, helping individuals prepare for discussions with healthcare providers. Coordinate and navigate health and care services for residents and their carers, working closely with social prescribing link workers, health and wellbeing coaches, and other primary care roles. Support the organisation and delivery of Care Home MDTs within the PCN. Demonstrate flexibility in liaising with multiple services and stakeholders, facilitating information-sharing between GPs, practices, social care teams, commissioners, patients, and the voluntary sector. Support the continued development of an integrated health and social care approach within the PCN, ensuring a coordinated response to referrals from GPs and health and social care teams. Act as a critical link in ensuring that patients are signposted to the appropriate health, social, or voluntary services in a timely manner.
