A part-time position working 12 hours per week, school hours over 2 or 3 days, has arisen to work with us in this rewarding role as Community Care Co-Ordinator.
Working as part of the practice team and closely with our Social Prescriber, you will be responsible for coordinating the resources available from within the community to proactively support those patients at risk of loss of independence as a consequence of unmet social care needs. We will support you to continue to develop collaborative working between the practice, the voluntary sector, community groups, the local authority, and volunteers, creating links and supporting growth and further development for the benefit of the practice's frail/vulnerable patients. Whilst the work will be predominantly with over 65s, the role will cover the whole range of the practice population.
This position would suit a graduate or complement and work alongside a similar part-time role.
Main duties of the job
Working predominantly in the practice, there may be times when you need to develop relationships and support local groups that will benefit the practice population by attending various meetings. You will manage your own diary and be responsible for your outcomes to support the role. Supporting patients with long-term conditions, helping patients to take control of their own decision-making and to maximise their levels of independence. Your role will include but is not confined to speaking to carers, patients, local community groups, volunteers, food banks, and Severn Hospice.

Job responsibilities
* To develop and maintain a detailed knowledge of local community and voluntary sector services to enable signposting of people at risk of loss of independence. Become the practice expert on what’s happening in the local community.
* To develop relationships for the benefit of the practice population with community groups/organisations and health and social care bodies.
* To provide an ambassadorial function for the practice in its relationships with the voluntary sector, volunteer networks, community groups, and all relevant statutory agencies.
* Working with the practice and community staff, utilising risk stratification information, to identify people at risk of loss of independence or admission as a result of inadequate social support.
* Where deficits are identified, support the community in the development of peer groups, carers groups, and volunteering including the compassionate communities movement.
Person Specification
Qualifications
* A good standard of general education: GCSE Mathematics C or above or equivalent, GCSE English C or above or equivalent.
* Experience of working with the general public.
* Experience of the voluntary sector, helping organisations.
* Experience of working within General Practice.
