Job Overview
The Weekend On Call Care Coordinator plays a vital role in ensuring that the HHC Care Team is supported in the community and that all domiciliary care calls are covered. The HHC On Call service is a telephone service available between 6 a.m. and 10 p.m.
The ideal candidate will possess strong organisational skills and a keen attention to detail, ensuring that all aspects of care are managed efficiently.
Working Schedule
Whilst you will be home-based, you will need to be available throughout your shift to support your team and deliver care in the community when required.
Duties
* Serve as the primary point of telephone contact for Carers and Clients.
* Address inquiries and concerns regarding care services.
* Update schedules to cover any sickness or absences, liaising with the Care Team.
* Community Care - Including Personal Care, Medication, Food, and Drinks.
Essential Criteria
* Previous experience in domiciliary care.
* Minimum 3 years of care experience.
* Knowledge of scheduling.
* Strong organisational skills.
* Understanding of safeguarding and how to respond.
* Excellent communication skills, both verbal and written, with a compassionate approach to client interactions.
* Proficiency in using office software.
* Ability to work collaboratively within a team while also being self-motivated.
* A commitment to maintaining client confidentiality.
Desirable Criteria
* Level 3 Qualification in Health and Social Care.
