Job summary East and West Neighbourhood Mental Health Teams are looking for an Advanced Clinical Practitioner to join each their teams as part of the Community Mental Health an Wellbeing Service. The Community Mental Health and Wellbeing Service (CMHWS) provides support, care and treatment to individuals in the community to holistically meet the wider range of mental health and wellbeing needs. The CMHWS comprises of two core pathways: the Neighbourhood Mental Health Pathway and the Community Mental Health Pathway (CMHP) This role will work across the CMHWS as part of an Integrated Model of Care. The post holder will provide advice, support, and treatment as part of the CMHWS, for members of our community experiencing problems with their mental health needs. Main duties of the job The Advanced Clinical Practitioner (ACP) role is undertaken by experienced registered health and care practitioners who have a high degree of autonomy. The ACP is required to provide advanced clinical advice, leadership, assessment, examination, diagnosis, and treatment within an agreed scope of practice. The ACP will demonstrate high levels of clinical judgement, critical analysis, and advanced decision- making skills at master's level. The post holder will support new ways of working that emphasise more efficient and patient focused services, and will ensure safe, high quality clinical care and excellent patient/service user experience.
