PURPOSE OF THE ROLE: The post holder will communicate and develop strong links with clinical services staff across the trust, as well as colleagues from external organisation such as Social Services, Mental Health Trusts, other ICBs and acute trusts, independent contractors and private care homes and providers. There will also provide contact with patients, carers and families. KEY RESPONSIBILTIES OF THE ROLE: To undertake Continuing Healthcare (CHC) and Funded Nursing Care (FNC) assessments and reviews on patients who may be eligible or who may already be in receipt of funding this will include those in receipt of fast track funding at end of life and those in receipt of a Personal Health Budget. To provide and receive highly complex, sensitive and contentious information, communicating unwelcome financial decisions to members of the public and using interpersonal skills to overcome barriers to acceptance where there may be hostility and antagonism. To case manage individuals in receipt of NHS funded care and support, reviewing and reassessing needs, the service provision and support required, this will include those in receipt of a Personal Health Budget. To review and reassess individuals eligibility for NHS CHC, FNC and any other NHS funded placements To undertake required assessments in support of the Deprivation of Liberty Safeguards (DoLS), this may form a key part of the role. To review any service, and care, commissioned in regards to quality and appropriateness to the individuals need, to ensure that the provider is delivering the requirements of the contract To complete retrospective reviews as required and directed by team manager To be confident in presenting cases and making recommendations on eligibility and care/support required To attend appeal/review panel meetings and present cases as appropriate To participate in case conferences as required. Ensure accurate minutes and record of meetings are kept and distributed as appropriate To participate in Best Interest Meetings as necessary To ensure accurate and open communication with a range of organisations and individuals To keep accurate and robust records at all times To work with members of the team and key stakeholders to ensure prompt delivery of the service within defined timescales To have the ability to communicate tactfully and diplomatically contentious information to various individuals To able to communicate with individuals who may have a language, sensory or cognitive impairment To able to discuss end of life issues with individuals/family in a sensitive manner To co-ordinate and monitor individual contracts for packages of care To undertake audit relating to CHC activity To implement local policies, systems and processes around CHC, Fast track as directed with reference to the relevant key national and local priorities To promote excellent customer care throughout the CHC processes ensuring local resolution of any concerns where possible. This may include providing and receiving highly complex, sensitive and contentious information requiring agreement or co-operation where there are barriers to understanding. This relates to external organisations and the public as well as to staff of the ICB and local authority To ensuring effective timely communication and liaison with key stakeholders, including partner agencies, GPs, patients and their families, and care providers To working with CHC Service Leads and cascade information as appropriate To be responsible for providing first level contact for disputes between partner agencies or with patients / families / legal bodies, for example following unwelcome financial decisions To implement patient / carer involvement at all stages of the CHC processes To ensure that the local authority is involved appropriately in the decision making process, and informed of the outcome where appropriate To promote awareness of NHS funded care To act as an expert resource to health and social care workers, patients, families and other organisations and individuals as appropriate To support CHC Lead to Investigating and responding to any complaints To have knowledge across a wide range of work procedures and practices related to CHC To deliver expert clinical and non-clinical advice as necessary To undertake necessary training To provide information relating to the monitoring of CHC activity and contract compliance To liaise is an integral part of the role and will take the form of regular contact by telephone, face to face meetings, written and electronic communication. To be responsible for ensuring value for money in all matters relating to packages for continuing NHS healthcare.
