Main duties of the job
You will work as an autonomous practitioner within a dedicated Multidisciplinary Hub. If you are committed, relish new challenges and strive for excellence, this post is for you.
You will hold a caseload and work within the principles of the agreed model of care for the service as laid down in the Operational Policy.
This will involve the assessment, care planning, implementation and evaluation of the care provided and will liaise with all agencies involved with the clients. The role includes being part of a duty rota, screening referrals and participating in Quality Improvement (QI) projects within the Team.
You will also take part in the Transformation Agenda in line with The NHS Long Term Plan, which describes:
New community-based offer which will include access to psychological therapies, improved physical health care, employment support, personalised and trauma-informed care, medicines management and support for self-harm and co-existing substance use and proactive work to address racial disparities.

Job responsibilities
* Comprehensive needs assessment and care planning ensuring this is holistic, person centred, co-produced utilizing DIALOG+
* Specialist clinical assessments and care delivery in line with best and evidence-based practice with reference to NICE recommendations

* Safeguarding awareness, alert raising and acting as enquiry officer
* Awareness and implementation of MHA and relevant legal frameworks. Good report writing and presentation of case

Education/ Qualification/ Training
* Registered Nurse (Mental/ Learning Disability Nurse)
Experience
* Working with people with mental health problems/learning disabilities within acute/forensic setting
* Managing challenging and aggressive behavior.
* Working within a multidisciplinary team environment
* Experience of working in a multi-cultural environment and knowledge of methods to develop culturally sensitive practice
* Experience of setting therapeutic boundaries with patients who exhibit challenging and high-risk behaviours, that require advanced interventions to enable effective de-escalation
* Experience of working with people who have personality disorders
* Experience of using creative and sophisticated interventions with patients in distress who use self-harm as a means of communication
* Experience of racial awareness/diversity training and the ability to operationalise this.
Ability and Skills
* Good verbal and written communication skills appropriate to the needs of patients within a learning disabilities service
* Ability to assess, plan, implement and evaluate care appropriate to the needs of learning disabilities patients using evidence-based approaches.
* Understanding of de-escalation in the context of acute or forensic nursing
* Able to work systematically and objectively in an environment with patients who can present high-risk behaviours
* Able to manage time so that deadlines are met
* Ability to recognize stress
* Ability to work as part of a team.
* Able to utilize a range of therapeutic interventions and frameworks to improve practice and service delivery
* Able to communicate with patients who are distressed, angry and have difficulty in communicating this.
