Job Summary:
The post holder is responsible for the delivery of general practice nursing services to the practice population and providing nursing assistance to all clinicians. Supported by the Head of Nursing, they will deliver care within the boundaries of their role, supporting patients to be healthy, monitoring long-term conditions, health prevention and screening activities. They will work collaboratively within the practice team to meet the needs of patients and support the delivery of policy and procedures.
Job Responsibilities:
1. Assess and deliver up-to-date travel health advice including the administration of vaccines under patient group directions and verbal and written advice on appropriate travel health information (e.g. malaria prophylaxis, bite prevention, safe sex, food hygiene, sun protection, etc).
2. Implement and participate in immunisation and vaccination programmes for both children and adults under patient group directions.
3. Review and document data using agreed READ codes to ensure accurate retrieval of information for monitoring and audit purposes.
4. Deliver opportunistic health promotion, facilitating change in a holistic manner to include smoking, alcohol, diet, and exercise as appropriate.
5. Assess and sign-post appropriately those patients presenting opportunistically with minor illness.
6. Document telephone and surgery consultations according to NMC guidelines.
7. Ensure awareness of statutory and local clinical protection procedures, including systems of referral and the ability to recognise signs and symptoms of child abuse.
8. Promote and deliver evidence-based care for patients presenting with aural conditions.
9. Promote and deliver the Cervical Cytology Screening Programme to all eligible patients.
10. Implement and evaluate personalised treatment plans for patients with long-term conditions in accordance with practice policies.
11. Perform venepuncture according to local guidelines.
12. Assess and deliver evidence-based wound care management in line with ICB and Practice formulary including the removal of clips and sutures.
13. Undertake Spirometry, ECGs, and Urinalysis as appropriate.
14. Prioritise health problems and intervene appropriately to treat the patient in emergency situations, including anaphylaxis according to the current UK guidelines.
15. Assist clinicians in the provision of minor surgery.
16. Obtain and document informed consent, verbal or written as appropriate including the use of Fraser Guidelines when necessary.
17. Apply infection control measures in accordance with local and national guidelines, including the annual infection control audit.
18. Ensure safe storage, rotation, and disposal of vaccines as per The Green Book.
19. Ensure stock of drugs and medical consumables are rotated and maintained to agreed levels.
20. Lead on Infection Control for the surgery.
Training and Personal Development:
1. Training requirements will be monitored by annual appraisal and will be in accordance with practice requirements. Personal development will be encouraged and supported by the Practice. It is the individual's responsibility to remain up to date with recent developments.
2. Annual appraisal will facilitate an assessment of your training needs and that of the practice.
3. Participate in the education and training of students of all disciplines and the introduction of all members of the practice staff where appropriate.
4. Undertake mandatory and statutory training.
5. If it is necessary to expand your role to include additional responsibilities, full training will be given.
Team Working:
1. Work closely with management, reception, and other staff to ensure the smooth running of the practice.
2. Maintain a good working relationship with other surgeries and agencies (e.g. Secondary Care).
3. Accept delegation from other nurses, prioritise own workload, and ensure effective time management.
Meetings:
Attend and contribute to various Practice meetings as requested.
