INTERVIEWS WILL BE HELD ON MONDAY 16TH DECEMBER
The post holder will work within their clinical competence as part of a multi-disciplinary team to:
1. provide expertise in clinical medicines management
2. provide structured medication reviews with and for patients
3. support patients in the management of their long-term conditions
4. oversee/support the management of medicines on transfer of care and implement systems for safer prescribing
5. manage repeat prescription authorisations and acute prescription requests
6. address the public health needs of the registered population
The post holder will perform medication reviews for patients with polypharmacy, especially older people, people in residential care homes and those with multiple co-morbidities; this may be in a practice setting or via phone or video consultation as agreed with the practice and the Lead Clinical Pharmacist.
The post holder will be involved in quality improvement and clinical audit as well as liaising with practices to manage some aspects of Quality and Outcomes Framework (QOF) and Investment and Impact Fund (IIF) work.
This role is pivotal to improving the quality of patient care and to developing operational efficiencies; it requires motivation and passion to deliver an excellent service within general practice.
Main duties of the job
The post holder will work in accordance with specific practice guidelines and protocols where applicable. He or she will be responsible for supporting network practices in implementing effective medicine management, identifying areas for improvement and initiating and managing change. Achieving clinical and non-clinical targets and objectives as agreed with the PCN and practices will be part of the role.
If you have not already, you will be required, and supported to complete the CPPE Clinical Pharmacist training pathway programme. If you have not already, you will be encouraged and supported to become an independent prescriber.
Job Responsibilities
1. Manage a medicines management plan and deliver patient services as determined by and with the Lead Clinical Pharmacist, practice / PCN policy and local and national guidance.
2. Work as part of the medicines management team within the PCN/practices and with medicines management colleagues within the ICB to acquire and share expertise and best practice to maximise cost-effective prescribing and improve the quality of patient care.
Engagement
1. Engage with PCN Clinical Pharmacist colleagues and the CD responsible for Clinical Pharmacy to contribute to the development of the PCN medicines management approach and team.
2. Perform clinical audits around key areas of responsibility.
Patient Services
1. Provide medication review services to patients via clinics in member practices, domiciliary visits and in residential and nursing homes, either face-to-face or via remote consultation technology and to deliver pharmaceutical care plans that maximise cost-effective prescribing and improve the quality of patient care.
2. Liaise with the PCN Education and Environment Lead (EEL) to arrange presentations at practice and patient group meetings or other events to give advice on the appropriate use of medicines.
3. Produce patient information leaflets and posters and run medicine awareness projects throughout the year.
4. Use evidence-based resources to support rational, cost-effective prescribing across the PCN to assist network practices with monitoring and management of their prescribing budgets.
5. Help plan, develop and support the introduction of new working processes within network practices to optimise the quality of prescribing.
Administration
1. Provide regular support and feedback to practices and, if appropriate, the network, on prescribing action plans.
2. Produce quarterly reports on practice performance on relevant medicines management measures for review by network member practices.
3. Update and maintain accurate patient records on the practices clinical record systems, including advice given and action taken.
4. Advise practices, and PCN colleagues if appropriate, on the safe and secure handling of medicines, ensuring compliance with medicines legislation.

The post-holder should appreciate the importance of effective communication within the team, particularly where there is a requirement to communicate both across the network and with individual practices with differing structures and needs. He or she will strive to:

Person Specification
Qualifications
* Professional registration with the General Pharmaceutical Council.
* Qualified from, enrolled or intention to enrol in an approved 18-month training pathway (e.g., CPPE GP, MOCH or PCN pathway).
* Be a prescriber, working towards or intending to become one within two years of start date.
* Post-graduate Clinical Diploma.
Experience
* Evidence of Continued Professional Development.
* Minimum two years relevant pharmacist experience post-registration.
* Good IT skills and literacy.
* Experience of partnership/collaborative working and of building relationships across a variety of organisations.
* Previous experience of working in primary care, in either a PCN or GP practice.
* Experience of working with GP prescribing systems, other clinical systems, and Microsoft Office.
* Experience of conducting medication reviews.
