Job summary Gillingham Medical Practice are looking for a highly motivated Frailty Practitioner to work within the Practice alongside our existing Advanced Nurse Practitioner and GPs. You will be required to work 25-30hrs per week. Main duties of the job You will be responsible for assessment and co-ordination of care and services to people with multiple co-mobilities and complex needs who live in their own homes and care homes. You will work autonomously, assessing individual patient needs, initiating investigations, with support determining a differential diagnosis and initiating appropriate holistic, evidence based treatment and care. As a Frailty Practitioner - you will be an experienced Registered Practitioner, who preferably has experience and knowledge around the care of older people and with patients with frailty and complex health needs.
