We recognise the importance of Clinical Pharmacists inPrimary Care and the key role that they have is supporting the delivery of theNetwork Contract DES and Quality Outcomes Framework (QOF). Delivering a rangeof health interventions at Practice level, including developing and managingmedicine optimisation services within Primary Care, systems for saferprescribing, structured medication reviews (for example of patients withpolypharmacy), reviews of patients with long-term conditions that fall withintheir competency, withdrawing medicines no longer needed through NHS ledprogrammes. For Enhanced Health in Care Homes residents will benefit fromregular clinical-pharmacy-led medicines reviews.
The ClinicalPharmacist plays a crucial role in our general practices and is of upmostimportance to our local patient population. It serves as a cornerstone in ourpursuit of enhancing health outcomes.
Main duties of the job
The post holder is a pharmacist, who acts within theirprofessional boundaries, supporting and working alongside a team of pharmacistsin general practice. In this role they will be supported by a senior clinicalpharmacist who will develop, manage, and mentor them and by a GP who willprovide clinical supervision as well as a line manager.
To provide clinical leadership on medicines optimisation andquality improvement as well as managing aspects of medicines safety andantimicrobial stewardship.
To work as part of a multidisciplinary team in a patientfacing role and take responsibility for areas of chronic disease managementwithin the practice and undertake clinical medication reviews to proactivelymanage patients with complex polypharmacy.

Job responsibilities
The post holder will provide primary support to generalpractice staff for prescription and medication queries. They will help supportthe repeat prescription system and medicines reconciliation on transfer of careand systems for safer prescribing, providing expertise in clinical medicinesadvice while addressing both public and social care needs of patient in the GPpractice (s).
The post holder will ensure that the practice integrates withcommunity and hospital pharmacy to help utilise skill mix, improve patientoutcomes, ensure better access to healthcare and help manage workload. The roleis pivotal to improving the quality of care and operational efficiencies sorequires motivation and passion to deliver excellent service within generalpractice. The clinical pharmacist will organise practice-based meetings withlocal community pharmacists to improve collaborative working and explore how tobest support patients to access and get the best from their medicines.
Person Specification
Qualifications
* Masters degree in pharmacy (MPharm) or equivalent
* Registration with General Pharmaceutical Council
* Post-graduate clinical diploma level or equivalent training/experience or independent prescriber
* A member of or working towards Faculty membership of the Royal Pharmaceutical Society
