The role of the Advanced Practitioner is as a non-medical practitioner, working as part of the Clinical Haematology team, under the supervision of a Consultant Haematologist.
The post holder will provide clinical expertise and advice for haematology oncology chemotherapy patients. They will demonstrate a sound understanding of the issues related to the identification, assessment, diagnosis, treatment and management of non-malignant and malignant haematological conditions and pre and post chemotherapy patients. Be involved in consultant clinics and assist in pre chemotherapy assessments and planning, and emergency attendance avoidance. They will have a key role within the MDT acting as a liaison between all members of the multi-disciplinary team, to allow for seamless and efficient care.
They will monitor and improve standards of care through supervision of practice, clinical audit, evidence-based practice, teaching and supporting professional colleagues and the provision of skilled professional leadership. They will participate in clinical governance within the service by undertaking audit and research as part of the MDT.
Under the direction of a Consultant Haematologist and following a programme of training and competency assessment the Advanced Practitioner will:
1. Provide advanced levels of clinical practice, knowledge and skills in Haematology.
2. Undertake Pre-chemotherapy assessment and physical examination as directed by the Haematology team.
3. Assess the need for and order investigations within scope of practice.
4. Have a clear understanding of their boundaries and know when to involve the Consultant Haematologist.
5. Manage medications including assessment, review and non-medical prescribing where appropriate and in accordance with legislation.
6. Manage own caseload with decision-making responsibilities e.g. fitness for treatment.
7. Undertake the process of gaining informed consent.
8. Prepare accurate and concise reports both verbally and in writing.
9. Perform some technical and invasive procedures according to their scope of practice (e.g., bone marrow biopsies).
10. Facilitate the continuity of care of patients.
11. Arrange appropriate pre and post chemotherapy investigations as part of the multidisciplinary team.
12. Liaise with accident and emergency for haematology emergencies and arrange admissions if required to maintain coherent service provision.
13. Provide ACP-led medication and review clinics.
The post holder will:
1. Attend mandatory training and reviews as local.
2. Promote and contribute to the development of the newly introduced role within the trust, being aware of role boundaries.
3. Actively engage in assessment and development of competence through observed and supervised practice.
4. Maintain high standards of care delivery through continuing professional development, evidence-based reflective practice and the maintenance of a professional portfolio and record of competency evidence and assessments, ensuring adherence to risk management and health and safety requirements.
5. Act as a resource and provide teaching, supervision, support and assessment to both doctors and nursing staff as appropriate.
6. Educate all colleagues both medical and nursing of the newly introduced.
7. Maintain and develop own professional knowledge.
8. Comply with the NMC/HPC codes of conduct.
9. Participate and lead in undertaking clinical audit.
10. Provide a visible clinical profile, having both an advisory and clinical input into patient care as directed by the Lead Clinician.
11. Post treatment care, including assessment, treatment review and identification of treatment induced problems and complications.
12. Deploy high level communication skills and effective communication strategies to ensure that information and knowledge is imparted sensitively according to the needs of patients, staff and students in training.
13. Support in-patients as a member of the MDT including attendance at board and ward rounds where required.
14. Evaluate care, including the discharge process and follow-up care arrangements for patients.
15. Keep relevant records and submit reports in a professionally acceptable manner.
16. Support patients through the emergency patient pathway from outpatient appointment to discharge following elective admission.
17. Be responsible for the emergency admissions and ensuring patients are being managed effectively along their pathway.
