Latrobe Regional Health (LRH) currently has an exciting opportunity for an experienced and dedicated Health Independence Care Coordinator to join our growing team on an ongoing, full-time or part-time basis, dependent on the successful candidate.
The primary purpose of the Health Independence Care Coordinator is to assess individual client needs and manage a caseload of clients accepted and enrolled in one or more of the following Health Independence Programs:
* Subacute Ambulatory Care Services (SACS), including Community Rehabilitation and Specialist Clinics.
* Transition Care Program (TCP).
About you
* Relevant professional qualification as either a Registered Nurse (Division 1).
* Registration with the Australian Health Practitioner Regulation Authority (AHPRA).
* At least five (5) years postgraduate experience in the Acute Nursing field.
* Evidence of recent relevant professional development.
