BradfordCity Primary Care Network 4 is looking to recruit a forward thinking andenthusiastic experienced Social Prescribing Link Worker.
Weare seeking to appoint a flexible minded Social Prescriber to take a front linerole working with the practices, supporting patients to the best possible careand support. This role will help empower local residents to take control oftheir health and wellbeing, allowing them to create time to focus on whatmatters to the patient.
TheSocial Prescriber supports taking a holistic approach to an individuals healthand wellbeing. The role will involve connecting people to community groups andstatutory services for practical and emotional support.
TheSocial Prescriber will provide support for low to medium intensity patients withmental and social health conditions to overcome barriers. The Social Prescriberwill be embedded within Primary Care Network multi-disciplinary teams toprovide personalised support to individuals, to enable them to take control oftheir wellbeing and improve their health outcomes.
Thesuccessful candidates will have excellent communication skills and a high levelof IT literacy.

Therole will help people manage their health differently and more effectively, theaim is to help people who are lonely or isolated, or who have complex socialneeds which affect their wellbeing, including their mental health.
Youshould be a good listener, have time for people and be committed to supportinglocal communities to care for each other. You should have experience of workingpositively with people facing complex social and emotional challenges.
Thesuccessful candidate should expect to work across all the practices deliveringcare to people from a wide range of backgrounds. At the centre of your workloadand commitment to patients is to work in a timely, efficient and confidentialmanner at all times.

Takereferrals from the PCNs Core Network Practices
Supportthe health and wellbeing of patients
Assesshow far a patients health and wellbeing needs can be met by services and otheropportunities available in the community
Co-producea simple personalised care and support plan to address the patients health andwellbeing needs by introducing or reconnecting people to community groups andstatutory services, signposting where appropriate
Evaluatehow far the actions in the care and support plan are meeting the patients healthand wellbeing needs
Providepersonalised support to patients, their families and carers to take control oftheir health and wellbeing, live independently, improve their health outcomes andmaintain a healthy lifestyle
Developtrusting relationships by giving people time and focus on what matters tothem
Takea holistic approach, based on the patients priorities and the widerdeterminants of health
Workin partnership with all local agencies to raise awareness of social prescribingand how partnership working can reduce pressure on statutory services, improvehealth outcomes and enable a holistic approach to care
Workproactively to develop relationships with external providers to facilitatejoint case management of patients accessing multiple services
Educatenon-clinical and clinical staff within the PCN on what other services areavailable within the community and how and when patients can access them. Thismay include verbal or written advice and guidance
Manageand prioritise their own caseload, in accordance with the health and wellbeingneeds of their population
Whererequired and as appropriate, refer patients back to other health professionalswithin the PCN.
