Principal Duties under direction of the daily clinical practitioner and supervised by the Operations Support Manager Administration Duties: Be the co-ordinator of planned activity for the management of frail patients. Ensuring that healthcare professionals focus their resources on those in most need, co-ordinating the right intervention for each patient and ensuring that records are shared appropriately. Use data to monitor and track activity to enable outcome reporting. Using this information to work on a daily and weekly basis to inform healthcare professionals to inform their caseloads. Attend the daily frailty multidisciplinary team meeting to represent the team and ensure a coordinated approach to the workload. Ensure that information from the MDT meeting is collated and captured accurately to inform decision making, clinical assessments and enable reporting. Take telephone messages and relay to the relevant person/team in a prompt, efficient manner. Liaise with General Practitioners, Practice Managers and other staff as appropriate. Help people to manage their needs, answering their queries and supporting them to make appointments. Support people to take up training and employment, and to access appropriate benefits where eligible. Carry out any other appropriate duties as required Management Allocate tasks on a daily basis as required as organisational priorities change as agreed with the Operations Support Manager Governance Assist in the collation of data for reporting purposes.
