Responsibilities:
1. Lead the effective implementation of the Trust's incident management and patient safety incident policies across the division of responsibility.
2. Escalate aspects of noncompliance with policy to divisional leadership.
3. Support staff in aligned services to report all new incidents and assign an investigator in a timely manner.
4. Support accurate scoring of risks and incidents and planning suitable actions to address reoccurring issues.
5. Provide expert advice and challenge to ensure that incidents are appropriately investigated and that areas for improvement are identified, with robust action plans in place to reduce the likelihood of recurrence.
6. Support patient safety investigations through all learning processes agreed by the Senior Business Partner.
7. Support Divisional staff to undertake immediate case reviews of patient safety incidents leading to (or potentially leading to) significant harm or significant learning.
8. Provide advice and solve problems or queries, and chase progress to ensure they are ready for presentation at the Directorate and Divisional quality meetings.
9. Support the collation of evidence that complete action plans.
10. Support the implementation of trust governance processes within the directorate and division.
11. Plan, deliver and facilitate improvement events and opportunities for experiential learning.
12. Ensure high-quality risk management is embedded, enhanced, and maintained.
13. Promote learning and development of all staff in risk processes and Trust risk policies.
14. Support the implementation of divisional risk management systems and processes.
15. Be a source of expert risk knowledge and advice for Divisional and directorate leaders.
16. Work with the Senior Business Partner to drive forward the quality governance agenda across the division.
17. Support the development of regular Divisional Risk Management Reports for the Trust Risk Management Committee.
18. Compile reports as required by the Senior Business Partner.
19. Ensure that processes for risk identification and escalation are working effectively.
20. Systematically analyse and review risk performance indicators including complaints, incident report forms, claims and litigation.
21. Support the continual improvement and management of the Divisional risk registers.
22. Review the directorate risk registers to look at trust trends and concerns raised across the Division.
23. Analyse Divisional and Directorate Risk Register submissions in conjunction with the relevant lead.
24. Support the integration of trust-wide risk management.
25. Forward plan risk management programmes across all directorates.
26. Liaise with staff across the Trust to ensure the timely compilation and production of risk registers.
27. Support the identification of gaps in control and assurance and develop action plans to address these.
28. Work collaboratively with staff to identify risk management priorities pertinent to particular areas or departments.
29. Support oversight of the Divisional management of complaints.
30. Monitor the divisional clinical outcomes processes including clinical audit, benchmarking and compliance with best practice.
31. Maintain oversight of the divisional compliance with the trust policies relating to clinical guidance.
32. Liaise with the central governance leads for clinical guidance.
33. Contribute to the delivery of corporate objectives on risk and patient safety.
34. Support the Senior Business Partner to promote quality and customer service, improving patient care by identifying areas of concern and repeated service failure.
