Job summary An opportunity has arisen in the Review and Resettlement Team based at Heathside Maidstone, for an experienced practitioner with a passion for supporting individuals with complex mental health needs, including autistic people. This role involves working closely with individuals accessing treatment in specialist hospitals outside the area and collaborating with a wide range of professionals to ensure high-quality care, effective resettlement, and seamless transitions. You will gain experience working with patients with complex needs, the CTR process, and working closely with external providers such as the Integrated Care Board (ICB), Social Services Kent, and Forensic to facilitate timely discharges from the hospital. This role allows you the flexibility of maintaining a good work-life balance with the option of working remotely. Main duties of the job To coordinate National Health Service (NHS) professionals for all health agencies involved. To be the main NHS point of contact for patients and their families who are admitted to an out-of-area hospital and receiving specialist treatment. To be the main NHS point of contact for the inpatient team and Community mental health teams To be the main NHS point of contact for the local authority. To liaise with and support other health professionals to ensure discharge actions are completed within the expected timeframes. To take responsibility, along with the Social Worker, for continuing progress toward achieving Resettlement Milestones. To report progress on the discharge actions to the OATS panel and KMPT forums that require updates. To work closely with the Social Worker to achieve timely discharges, identify potential barriers to discharge and identify solutions or escalate as appropriate. To consult the Chair of each patient's CTR regarding any issues the CTR Panel wish to highlight to the Discharge Planning Group.
