Job summary Average 37.5 hours per week. Day shifts and night shifts. Includes some weekends. An exciting new opportunity has arisen within The Community Rehabilitation Service. There is soon to be a new rehabilitation unit opening in North East Lincolnshire. The Community Rehabilitation Service supports individuals in their recovery from mental ill-health. The role of the Senior Support Worker is to work across the rehabilitation pathway (based primarily at Tees House), supporting in the assessment of the health, social and wellbeing needs of the individuals using the service as well as the delivery of appropriate interventions accordingly. The successful candidate will be able to work both autonomously as well as part of a multidisciplinary team (MDT), attending reviews, MDT meetings and other meetings whether clinical or not. Main duties of the job This role is focussed around spending time with our service users and as such it is a privileged role that gives you the opportunity to make a real difference to peoples lives. The role will support the multi-disciplinary team to delivery individualised care packages to a small group of service users with both mental health needs and/or a learning difficulties, who are either stepping down from low secure and locked rehabilitation placements or who are local service users requiring assessment and treatment for their enduring mental ill health. The aim of the post is to support recovery, independence, quality of life and well-being, through instilling hope and optimism in every service user so that they can reach their full potential.
