Role Summary
* To assess, provide and review nursing care, advice and information to patients and carers within the home environment or appropriate care setting in a safe and professional manner using evidence based knowledge.
* Aim to wherever possible maintain patients in their own preferred place of care, enabling them to maximise their independence and optimise their quality of life.
* To work under the direction of the Community Specialist Nurse and the Nurse Team Manager (Community or Hospital) using their skills and knowledge as a Registered Nurse.
* To be expected to hold a designated caseload and deputise in the absence of the Community Specialist Nurse.

In the Exmouth, Budleigh, Woodbury Cluster we have exciting opportunities for you to join one of our Community Nursing teams, We work in a coastal seaside area and look forward to you joining our Community Nursing teams covering 6 GP practices.
As a Registered Nurse working in the Community you will provide person centred care based on the holistic nursing assessment in order to plan, implement and evaluate nursing care. You will promote and support self-management and work with people to manage their long-term conditions, prevent hospital admission, and support early discharge from the in-patient setting. The role also focuses on using and developing your clinical skills and supporting individuals at the end of life. You will work in a Community Nursing Team which is part of the wider Adult Health and Social Care Team, as well as working with a wide variety of providers, to meet the nursing needs of individuals in their home setting or alternative residential settings.
Experience in Community Nursing is desirable but not essential as full training will be provided and fully supported.
We welcome applications from newly qualified Registered Nurses including those who are due to qualify in the next few months. The trust offers a comprehensive preceptorship programme to support this.
Within Community Nursing, career development opportunities include education in specialist areas, such as Tissue Viability, End of life care and Long-term Conditions, with link Nurse roles supported to enable continual professional development.
We also provide opportunities to study for the Specialist Nurse Practitioner qualification, which is currently being awarded over a two-year programme to successful applicants.

Working Pattern: 37.5 hours per week, Monday to Friday 7.5 hours daily, to include every 4th weekend

Pay: The salary advertised for this position reflects the Agenda for Change 2024-5 rates, inclusive of a 5.5% pay award implemented from October 2024. If you join before this date, the pay award will be backdated to April 2024 or your start date, whichever is later. Until then, remuneration will be based on the 2023-4 pay bandings (Pay scales for 2023/24 | NHS Employers ).
