Referrals and Assessments Coordinator (Part-time, 3 Days Per Week)
Location: Headway Milton Keynes, 518 Silbury Boulevard, Centre:MK, MK9 2AD, some travel may be required for appointments so access to own vehicle is desirable.
Working Days: Tuesday, Thursday, and Friday 9am – 5pm

Headway Milton Keynes is a small local charity supporting individuals who have suffered a brain injury through functional rehabilitation sessions. Headway Milton Keynes is affiliated to Headway UK, but operate independently and develop the service alongside community need.
Job Summary:
Headway Milton Keynes is seeking a highly motivated and organized individual to join our team as a Referrals and Assessments Coordinator (you may see/hear this referred to as RAC).
This part-time role is essential to ensuring smooth processes for referrals, assessments, client onboarding, and funding applications. The successful candidate will work closely with local councils and other agencies, coordinate care reviews, and contribute to key administrative functions within the organisation.
Key Responsibilities:
1. Referral Triage and Initial Assessments
o Review and triage incoming referrals from various sources.
o Conduct initial assessments to determine client needs and suitability for Headway Milton Keynes services.
2. Onboarding Process for New Clients
o Ensure a smooth and efficient onboarding process for new clients.
o Provide clients and their families with necessary information and support during the transition into services.
o Create and document appropriate information on Care and Support Plans.
3. Funding Applications
o Submit funding applications for new and existing clients as per their needs.
o Track and follow up on the progress of funding applications.
4. Liaison with Local Councils
o Establish and maintain strong relationships with local councils.
o Liaise and follow up on outstanding funding or support issues with local councils.
o Lead the discharge process for clients when necessary, ensuring clear communication and appropriate transition planning.
5. Care Reviews
o Conduct care reviews for existing clients to assess the continued appropriateness of services and ensure they are receiving the necessary support.
6. Also expected in the role: Team Collaboration, Fundraising Support (Ad-Hoc), and Cover for Rehabilitation Sessions (As Needed)
Essential Skills and Qualifications:
* Experience in a healthcare or social care setting, preferably in assessments or referrals is desirable.
* Strong communication and interpersonal skills, with the ability to liaise effectively with clients, families, and external agencies.
* Excellent organizational and time management skills, with the ability to prioritize tasks effectively.
* Ability to work independently and as part of a team.
* Strong attention to detail and accuracy in documentation.
Desirable Skills and Qualifications:
* Previous experience in a similar role within a charity or healthcare organization.
* Knowledge of brain injury and neurological conditions, or a willingness to learn.
Hours:
This is a part-time position, working 3 days per week 9am – 5pm (Tuesday, Thursday, and Friday), with occasional flexibility required for ad-hoc tasks or urgent needs.
