Job Overview: We are seeking an experienced Band 6 Occupational Therapist to join the team at Manchester University NHS Foundation Trust. This role involves providing specialist assessment and discharge planning support in an acute medicine environment. Key Responsibilities: Conduct comprehensive assessments to identify patients' needs within the acute medicine setting. Provide expert advice on rehabilitation and ongoing care plans to ensure seamless transitions. Maintain accurate and up-to-date documentation in line with Trust standards. Participate in team meetings, case reviews, and service improvement initiatives. Requirements: HCPC registration as an Occupational Therapist. Proven experience in acute medicine, focusing on assessment and discharge planning. Strong clinical reasoning and decision-making skills.
