Job summary Social Prescriber Link Workers play an important role within a Practice to proactively identify and work with people, to provide non-medical support that improves their health and wellbeing. The service will support patients to access all available opportunities by linking individuals with community and volunteering support so they can take charge of their own health and well-being. The role is an integral part of the Practice multi-disciplinary team and should address the wider determinants of health, such as debt, poor housing and physical inactivity. You will be required to provide personalised support to individuals, their families and carers to achieve better health outcomes. Main duties of the job You will be required to assess, plan, develop, implement and evaluate programmes & individual care plans that promote health, primarily by services & opportunities that are available in the community. You will be required to expand the referral gateway to take referrals from a wide range of agencies in line with Practice requirements. To promote social prescribing, its role in self-management & the wider determinants of health. Assess and integrate issues surrounding what the patient sees as important for them into the overall therapy process. Be proactive in developing strong links with all local agencies in line with the social prescribing implementation plan to encourage referrals recognising what they need to be confident in the service to make appropriate referrals. Produce a simple personalised care plan to address the patients health & wellbeing needs by introducing or reconnecting people to community groups or statutory services. You will be required to liaise with patients, their families/representatives, healthcare professionals, social services, GPs & other organisations to ensure that the care needs of the patient is being met & the desired outcomes achieved. A high level of communication is required to work with individuals to establish their goals.Coaching skills and motivational interviewing will be used to motivate, encourage & support individuals.Difficult conversations will be required to understand why individuals decline to work to the agreed programme.
