Job summary We are seeking to recruit an enthusiastic, highly motivated person with a background innursing, mental health, as an allied health professional or other relevant experience ofmanaging complex caseloads to provide holistic advice and support to patients both adultsand families with children who frequently urgent care services across West Kent. Main duties of the job This is an exciting role within a successful service, which provides the appointed post holderthe opportunity to be innovative and shape service delivery. The post holder will work in amulti-disciplinary approach across all providers. This role will entail identifying the unmetmedical, psychological and social needs of patients which have led them to access urgentcare services frequently. These needs will be addressed in a number of different waysincluding actively listening to the patient's concerns, without judgement or sanction, de-escalating evolving crises of psychological or physical health and acting as a liaison betweenthe patient and services within the community, to reduce the risk of crises developing.Experience of working with people with complex physical, psychological, emotional, and socialneeds is required alongside a high degree of self resilience and the ability to workautonomously.The post holder must have a full driving license and will be required to travel to variouslocations in West Kent to meet with patients.
