We are looking to increase our existing team The role is for 25 hours per week on a fi xed term basis for 12 months. The salary shown above is based on a full time role of 37.5 hours, therefore the actual salary pro rata to the 25 hours would equate to £16,848.50 per annum. Main duties of the job The Care Coordinator will work with the network in supporting them with the NHS England Early Cancer Diagnosis DES. A large part of your role will involve supporting patients in the early stages of their cancer diagnosis through treatment and living with cancer. The role is key to supporting patients after they have been diagnosed with Cancer and helping them to understand the implications of their diagnosis and what the next steps are, matching the support that is available locally to help patients during this worrying time. Work closely with Trust colleagues to ensure that the patient attends the appointment.
