To lead, deliver and support ongoing development of a clinical pharmacy service within the Community Response Service (CRS) and the Community Services Business Unit (CSBU) of the Northumbria Healthcare NHS Foundation Trust. To provide excellent patient care through direct patient facing activities as part of clinical pharmacy service delivery within CRS. To provide leadership on matters pertaining to medicines use and optimisation within the CRS (and in support of the CSBU). This includes providing advice on medicines use (risk, clinical governance, research and development, formulary, expenditure) and to contribute to the development of relevant protocols and guidelines. To undertake risk management and clinical governance activities within own assigned areas of responsibility Pharmacy services provided within the Community Services Business Units, within which the Community Response Service falls. This role encompasses the 2hr Urgent Response Service, Admission Avoidance, Proactive Care, Community Therapy and provide support to Virtual Wards when necessary.
