Job Overview
We are looking for a proactive and organised Care Coordinator to join our team at St Peters Surgery, supporting our patient-centric, integrated care model within the Woolston and Townhill PCN. This role is crucial in coordinating patient care, particularly, in the management of vaccinations and long-term condition (LTC) clinics. The successful candidate will work closely with primary care and multidisciplinary teams to improve patient outcomes and promote independent health management.
Key Responsibilities
* Clinic Coordination: Coordinate clinic schedules and manage clinic resources.
* Vaccination Management: Oversee vaccine stock, scheduling and movement to ensure efficient and accurate delivery.
* Data Management: Maintain and analyse data, particularly in Excel, to support patient care, medicines management and clinical operations.
* Communication: Manage and execute social media campaigns, website and patient communications.
* Task Ownership: Demonstrate initiative by working independently, organising daily tasks, and proactively managing responsibilities with minimal supervision.
* Reporting and Feedback: Gather patient feedback and provide feedback on clinic operations to management and participate in quality improvement activities.
* Patient participation group: Coordinate and manage the group.
* Meetings: Coordinate, manage and minute practice meetings.
* Regulatory: support CQC, regulatory and contractual processes with clinical and management leads.
* Reception: support reception and admin teams as required.
Essential Qualifications and Skills
* Organisational Skills: Highly organised with strong time management abilities to handle complex schedules and multi-task in a busy environment.
* Technical Proficiency: Advanced knowledge of Microsoft Excel and other digital tools for managing databases, appointments, and clinic workflows.
* Interpersonal Skills: A people-oriented individual with excellent communication skills, able to work with patients, carers, and multidisciplinary teams.
* Self-Directed: Able to work independently, take initiative, and complete tasks without constant direction.

Pay: £24,000.00 per year
