An exciting opportunity has arisen in our team. We are looking for an experienced nurse who is eager to learn, cardiology or anticoagulation experience is not essential as we will provide training.
The role would suit someone with excellent communication skills, who is keen to provide education and motivate patients whilst providing high quality clinically effective care.
Main duties of the job
To work under instruction from the senior staff in cardiac rehab team to:
* Provide an evidence-based phase I-III cardiac rehabilitation in line with all national and local guidelines.
* Take an active part in the exercise and secondary prevention components of the cardiac rehabilitation service held at multiple sites in the Medway area.
* Under the guidance of specialist nurse assist in answering correspondence and telephone calls from cardiology patients, to provide advice, support and follow up.
* Visit cardiology patients in their homes or in clinic to review their progress, facilitate medication optimisation and support lifestyle changes.
* Contribute actively to the monitoring and audit of the cardiac rehabilitation service.
To work as a member of the anticoagulation team to:
* Provide care to patients taking warfarin or other vitamin K antagonist oral anticoagulants in their homes or in clinic.
* Assess patients, plan and implement care that involves undertaking near patient blood sampling (finger pricking) to obtain INR reading and dosing patient’s warfarin as appropriate following protocols.
* Assist in the coordination of the workload of the team, ensuring the delivery of an effective service and allocation of resources.
* Assist on the development and implementation of evidence based clinical practice and service delivery.
* Contribute actively to the monitoring and audit of the anticoagulation service.

Person specification
Qualifications
* Registered General Nurse (Part 1 of the NMC registered)
* Nurse education to degree level or currently on degree pathway
* Degree level in anticoagulation management theory and practice or willingness to undertake
Experience
* Experience post registration required
* Evidence of continuing professional development
* Community nursing experience
* Experience of health promotion
* Has experience of autonomous working.
Special knowledge / expertise
* Experience of working with patients who have had myocardial infarction or cardiac surgery
Disposition, adjustment, attitude and commitment
* Team worker committed to ensure that patients receive an excellent and high standard of service.
Practical / intellectual skills
* Ability to travel to various sites in a timely manner
