An exciting opportunity has arisen for a band 5 to join the SDEC and MDU teams.
This 30 hours a week post will cover both units.
Within SDEC the post holder will participate in the rapid clinical assessment of acute medical patients. The post holder will help to plan, co-ordinate, implement and evaluate care and assist in the coordination of the patient's journey in order to minimize waits, delays and prevent avoidable admissions.
The Medical Day Unit (MDU) is a Nurse Led, medical infusions, investigations and procedures unit accepting elective patients from medical specialities.
Main duties of the job
1. To maintain accurate, legible, contemporaneous records of care given.
2. Record keeping and ensuring all documentation is completed.
3. Responsibilities for ensuring patients receive high standards of care.
4. Evaluating clinical care and acting upon the outcomes of evaluation.
5. Ensuring that equipment is safe for use and report any failures in accordance with agreed department protocols.
6. Keeping up to date with clinical developments and standards eg: NICE guidance.
7. To be involved in the evaluation of clinical care and the development of actions plans to address any deficiencies.
8. To maintain a portfolio of evidence of clinical skills, competency based assessments and underpinning theoretical knowledge as required by the NMC.
9. To work in a professional autonomous manner within agreed clinical guidelines, accepted both locally and by governing bodies.
10. Liaise with various staff across the Trust to ensure patients' care is of a high standard.
11. To assess and carry out treatment/ investigations that are required.
12. Follow pathways and protocols and evidence your work.
13. You will be expected to work as part of a team as well as independently.

Job responsibilities
1. Act as a support/mentor for other staff within the Department.
2. In conjunction with other staff members, plan and prioritise own workload.
3. Self-development and participation in continuous and post-basic education, as laid down by the NMC.
4. Maintain standards and work to clinical nursing procedures as formulated by the Trust.
5. Assist in the induction training of new staff members, meeting professional requirements by actively participating in teaching.
6. To be involved in the mentorship of new staff members and the development of a comprehensive training package.
7. To act as an expert resource for training/education for other team members.
8. Maintain up to date knowledge of developments in nursing.
9. Ensure that patients are kept informed at all times.
10. Ensure that patients have their dignity and privacy maintained at all times.
11. Report for duty attired and of appearance so as to conform with Trust policy.
12. Learning from the patient experience to constantly drive up standards is paramount. The post holder will be accountable for ensuring the highest standard of care is delivered and will constantly promote: patient safety, effective care and a positive patient experience at each stage of the patients journey.
Person Specification
* NMC registered
* 2 years post graduate experience
* Extended skills - Phlebotomy, cannulation, ECG and IV medications
* Evidence of ongoing professional development
* Leadership Skills

Salary: £29,970 to £36,483 a year per annum pro rata
