About the Team
Warwickshire County Council is thrilled to announce an exciting position within our services! We are on the lookout for a passionate Social Care Worker to join our dynamic Reablement team. Our well-established team includes Social Care Practitioners, Occupational Therapists, Occupational Therapy Assistants, Reablement Officers, Reablement Supervisors, and Reablement Assistants, all dedicated to achieving the best outcomes for our customers on their Reablement journey.
This is a fantastic opportunity to become a Social Care Practitioner in our county-wide Reablement Service. In this role, you will assess customers in their own homes, following a therapy-led care package provided by our team. We pride ourselves on a holistic approach that embraces multi-professional skills, empowering customer choice and control.
You will work with customers aged 18 and over, addressing a range of functional difficulties, whether cognitive, physical, environmental, or psychological. While the role is county-wide, you will have a base in either the North or South, with the flexibility to support the Reablement Service across the county during busy periods. Additionally, you may assist the Hospital Teams when needed, with full support provided.
Our team also supports the Moving on Bed Service (MOB), which includes Residential settings and Extra Care Housing (ECH). These services facilitate hospital discharges for a six-week period, offering further assessment away from an acute setting.
You'll enjoy a vibrant work schedule, Monday to Friday, with the added excitement of working one Saturday every seven weeks. Plus, you'll have the unique opportunity to work on bank holidays on a rota basis. Join us and make a real difference in the lives of our customers!
