Job summary The post holder is responsible for the delivery of practice nursing services and care to the practice population. Supported by their peers and the Nurse Manager within the practice, they will deliver care within the boundaries of their role, focusing upon supporting patients to be healthy, monitoring of long-term conditions, health prevention and screening activities. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. Driffield is a market town in the East Riding of Yorkshire surrounded by the picturesque wolds, with easy links to nearby York and Beverley and is approximately 10 miles from the beautiful Yorkshire coastline. The main surgery is located in Driffield town centre with a branch surgery in the village of Wetwang 6 miles west of Driffield. We are a semi-rural practice with 12,300 patients. We dispense to approximately 50% of our patients from Driffield and Wetwang. The practice is a young enthusiastic team which comprises of 2 partners, 5 salaried GPs, 3 ANP's with support from the nursing and HCA team. We are a training practice with GP trainees from ST1-ST3.
