We are seeking a talented, dynamic and experienced clinical lead to join our enhanced clinical triage and case coordination function in our established Single Point of Access. Initially, this function will support community nursing, urgent care services, and end of life care with consideration of expanding over time as the model develops. You will work across multiple sites in Hounslow, so the ability to travel efficiently between sites is crucial. You will possess excellent organizational and people management skills and be experienced in clinical risk management and collaboration/negotiation.
In your supporting statement, please relate your skills, experience, and knowledge to the person specification and in particular to your abilities in clinical triage and decision making in the services within scope for the Clinical SPA.
