The Urgent Community Response (UCR) Sister will continuously assess the needs of individuals with long-term conditions, complex health needs, and acute episodes of ill health requiring an urgent 2-hour response, and agree care plans, utilising the single assessment process.
They will work with area professional leads and in partnership with GP practices and local Primary Care Networks to further develop services that are valuable to the commissioners and meet the local needs of the population.
The post holder will work autonomously as part of the Community UCR to provide a comprehensive assessment and patient-centred care package, utilising support services and liaising with other agencies to ensure the needs of this client group are met.
The UCR Sister will have physical assessment and disease management skills and will evidence management of acute/chronic clinical conditions, multiple co-morbidities, and poly-pharmacy within the home environment, ensuring an optimum standard of patient-centred care.
Successful candidates must have access to a vehicle for work purposes to meet the requirements for the role.
Main duties of the job
* To be responsible for monitoring and supporting patients under the care of the UCR team (previously known as Rapid Response/Fast Response caseload) to provide appropriate management, treatment, and advice to patients and provide clinical support for the associated nursing and therapy staff.
* To have the physical assessment and disease management skills to deliver an effective clinical assessment providing an urgent crisis response (UCR) to all patients in the Redcar and Cleveland, Middlesbrough, and Hambleton and Richmondshire localities within 2 hours of initial referral.
* Working autonomously, the post holder will have continuing responsibility for pro-actively monitoring and supporting patients within the UCR service.
The post holder will be expected to work flexibly across 7 days a week, ensuring the 24-hour service is covered to meet the changing needs of the service, which may include working across the Middlesbrough, Redcar and Cleveland, and Hambleton and Richmondshire areas.
This role involves both day and night shifts.

Person Specification
Qualifications and Training
* To hold a diploma/Degree awarded by a professional body.
* Registered as fit to practice by professional body, e.g. Nursing and Midwifery Council NMC/HCPC as a Nurse/Physio/OT/Paramedic.
* To be an independent non-medical prescriber or be willing to work towards within the first 12 months.
* To have completed the physical assessment and disease management skills course (level 6).
* Evidence of Post Graduate clinical training/development.
* Professional knowledge acquired supplemented by post-graduate diploma specialist training, experience, short courses plus further specialist training to degree equivalent level.
* Specialist knowledge, underpinned by theory and experience.
* Long Term Conditions modules relevant to adult patients.
* Teaching and mentoring qualification.
Experience
* Demonstrates clinical knowledge and skills relevant to the role.
* Evidence of MDT working.
* Evidence of post-graduate training/experience in leadership.
Personal attributes
* Willingness to be challenged.

Salary: £37,338 to £44,962 a year (pro rata)
