The post holder will work closely with staff working at practice and PCN level to support population health management, improvements in care quality and PCN operational efficiency. Havering Liberty PCN is led by our Clinical Director and PCN Strategic and Transformation Manager. Our team currently includes Clinical Pharmacists, Advanced Clinical Practitioners, Dieticians, Social Prescribers, Physiotherapists, Care Coordinators, General Practice Assistants, Nursing Associates and Pharmacy Technicians. The PCN also works closely with and is supported by its GP Federation, Havering Health Ltd, who are the employing body for the workforce of the network.
