Duties and responsibilities will include: Using GP systems to identify the cohorts of patients who require specific health checks Coordinating the booking of identified patients into the practice clinics (telephone or IT remote booking system) To regularly monitor the appointment systems to ensure all appointments are utilised and (working under the supervision of the Lead Care Coordinator) to take action to book all available appointments Supporting admin teams in practices to telephone patients that are due for a health check / review or have not attended a booked appointment To support practices in coordinating the vaccination of care home and housebound residents (including the preparation and coordination of patient lists, planning visits and communicating with patients) To review uptake of appointments and (with support from the Lead Care Coordinator) to identify at risk groups that need to be prioritised and contacted. Any other duties as requested by the Lead Care Coordinator or Management Team
