Main duties of the job
CareNavigator duties
Answerthe telephone promptly and deal with queries appropriately.
Makeappointments with GPs, nursing staff and GP Federation/ Primary Care Networkstaff.
Signpostpatients to more appropriate services when necessary
Preparerepeat prescriptions ready for GPs to check and sign.
Dealwith patient requests regarding results of diagnostic tests
Administratorduties
Recordand maintain accurate patient information, summarising and coding informationonto the patients notes using the practice computer system.
Check,scan and electronically file correspondence from the local hospitals andcommunity services

Job responsibilities
To be responsible for undertaking a wide range of reception duties and the provision of general support to the multidisciplinary team. Duties can include but are not limited to, projecting a positive and friendly image by greeting and directing patients and other visitors, effective use of the appointment system, booking appointments, processing of information and assisting patients as required. To act as the central point of contact for patients, the distribution of information, messages and enquiries for the clinical team, liaising with multidisciplinary team members and external agencies such as secondary care and community service providers. Primary Responsibilities: The following are the core responsibilities of the care coordinator. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels: a.Maintaining and monitoring the practice appointment system.b.Process personal, telephone and e-requests for appointments. incoming phone calls, transferring calls or dealing with the callers request appropriately.d.Signpost patients to the correct service.e.Initiating contact with and responding to, requests from patients, team members and external agencies.f.Process and distribute incoming and outgoing mail. incoming emails when necessary.h.Complete opening and closing procedures in accordance with the duty rota and maintaining security in accordance with Practice protocols. required support the management of repeat prescriptions, ensuring they are processed accurately and efficiently in accordance with practice guidelines.j.Photocopy documentation as required requests for information SAR, insurance / solicitors letters and DVLA forms to the administrative team.l.Clearing and re-stock consulting rooms as required. all queries as necessary in an efficient manner.n.Maintain a clean, tidy, effective working area at all times.o.Monitor and maintain the reception area and notice boards, keeping the reception area free from clutter and obstructions.p.Support all clinical staff with general tasks as requestedq.Provision of refreshments for staff and visitors as required.
