We are looking for an experienced and motivated clinical pharmacist to work in our AAU Pharmacy team. The AAU Pharmacy team play a vital role in medicines optimisation as key members of the multidisciplinary team on the unit. You should be an innovative and enthusiastic team player who communicates effectively at all levels, and should hold a post-graduate clinical qualification and significant relevant clinical experience of working on a busy hospital ward and as part of a multidisciplinary team. Provide excellent patient care through direct patient facing activities. Provide a 7 day pharmacy service as per agreed shift pattern (12 hour shifts) Support transition of care of patients from AAU to other wards. Provide leadership on matters pertaining to medicines optimisation, to advise on medicines use (risk, clinical governance, research and development, formulary, expenditure) and to develop/audit relevant protocols and guidelines. Be responsible, on a day to day basis, for medicines reconciliation and medicines optimisation for patients on AAU Provide a clinical service and support to SDEC (Same Day Emergency Care) unit Undertake clinical supervision and training of junior staff Provide support and in-reach to other ward areas where necessary Our Organisation The Princess Alexandra Hospital NHS Trust (PAHT) provides a full range of general acute, outpatient and diagnostic services at The Princess Alexandra Hospital in Harlow, the Herts and Essex Hospital in Bishop’s Stortford, and St Margaret’s Hospital in Epping.
