Job overview
We are looking to recruit a Critical Care Outreach Practitioner on a permanent basis. We are looking for a compassionate, enthusiastic and driven individual who is passionate about making a difference to the care of acutely unwell and deteriorating patients.
Our CCOT is a proactive service that provides early recognition, assessment and management of the deteriorating patient trust wide.
As a CCOP you are responsible for the delivery of patient focused programmes of care, which addresses the complex and changing health and well being needs of the deteriorating and critically ill patients throughout the trust. You will be part of a team at the forefront of developing a service to meet the future needs of the Trust.
Main duties of the job
* Provide effective and efficient communication and working relationships with nursing colleagues and MDT members throughout the wards and departments and other hospitals when discussing clinical situations and when attending meetings.
* Toutilise a variety of strategies to communicate highly complex issues that may affect acutely ill ward-based patients and those involved in their care.
* Provision of a rapid response and support system to allow the use of expert knowledge and in depth experience to identify deterioration of patients’ physical condition from the NEWS system and as a result provide highly specialized intervention and advice with appropriate physical assessment, observation and treatment for acute patients on the ward.
* Act as an expert resource and point of referral for nursing in the specialty and advocate on behalf of critically ill patients, their families and the service as a whole.
* Need to prioritise own case load with regards to the follow up of patients following discharge from the critical care unit.
* Receives referrals from all wards within the Trust. Discusses care of any critically sick patients and advises on treatment within the agreed protocols. Summarises all patient assessment in medical notes and advises staff of recommendation made. Ensures that further referrals are made to other specialist teams where appropriate.

Detailed job description and main responsibilities
* To provide expert advice, support and direction to ensure holistic patient care for acutely ill and deteriorating ward patients in conjunction with the multi-disciplinary team.
* Work closely with the Critical Care Nursing and Medical Teams to facilitate early and timely admission to/and safe discharge from critical care.
* To assist providing the infrastructure that addresses the knowledge and skills deficits of ward teams in delivering acute care skills to patients.
* Liaise closely with the RaCI assistants to support the ongoing rehabilitation needs of patients post-discharge from critical care.
* Institute national guidelines or evidence based practice to improve patient care.
* Addresses clinical governance issues promptly.
* Maintains a high profile and publishes work locally, nationally as appropriate.
* To role model compassionate and inclusive leadership in order to shape the creation of a collective leadership culture within the trust. This means demonstrating a consistent leadership style which (a) engages, enables and empowers others (b) uses coaching to promote ownership of learning and quality improvement and (c) facilitates team working and collaboration within teams / departments and across organisational boundaries.

* Degree or equivalent experience
* Formal Teaching qualification ENB 998/ C&G 730 or P126/127
* Advanced Life Support/Intermediate Life Support

* Significant experience in critical care/acute care environment
