We are looking for a forward-thinking and motivated nurse to join the Neighbourhood Nursing teams across Lambeth and Southwark areas.
We are the first Trust in the UK to trial the Buurtzorg District Nursing Model: an entirely Nurse-led approach to patient care in the community with self-managed nursing teams. You will find yourself working in a completely different way.
You must demonstrate excellent communication and team working skills. Through clinical case management you will care for patients with long-term conditions, those at risk, end of life care needs, and those with supported self-care programmes.
There will be training and opportunities in addition to clinical supervisions and appraisals. The post holder must be able to undertake a prescribing course and advanced assessment courses once in post.
Working on a 7-day shift service (hours between 8.00 – 20.00), this post also includes rotational weekend work (maximum two per month).

The Neighbourhood Nurse participates in the clinical case management of patients who have a wide variety of complex, high-risk and supported/self-care conditions.
As a team leader, you will ensure the provision of a high standard of nursing care and health promotion.
Duties include:
• Holistic assessment, planning, implementation and evaluation of care. For example, Chronic Disease Management, Tissue viability, Palliative Care, Nurse Prescribing, Extended and Supplementary Prescribing.
• First contact assessment, care and referral.
• Continuing care, rehabilitation and delivering the National Service Frameworks.
• Providing public health, health promotional programmes to improve health and reduce inequalities.
• Maintaining contemporaneous written and computerised records.
