About the role
We are recruiting a full time Assessment Support Coordinator to work within the Home First (hospital discharge) Team in West Dorset, based in Dorchester.
As an Assessment Support Coordinator, you will be working alongside Social Workers, Occupational Therapists, and health colleagues across a range of settings to support people to remain as independent as possible and linked in with their local networks.
* Undertake strengths-based assessments and support planning, in accordance with the Care Act 2014.
* Assist individuals to identify their goals and how best to achieve them to support their independence, and where appropriate complete mental capacity assessments.
* Work as part of a busy team and alongside other teams within the local authority. Multi-disciplinary practice will be integral to the role.
Our Home First Team has a strong track record in supporting new starters to Dorset Council Adult Services by being friendly and supportive, developing individuals and offering a detailed and comprehensive induction plan.

About You:
* Excellent communication skills, enjoy working in a fast-paced environment, have a flexible approach and be able to work closely with a range of professionals.
* Experience of working in a social care setting where you have supported people to maximise their potential, but applications from candidates with transferable skills are also welcomed.
* Confident in meeting new people and able to adapt to work across a range of social care settings.
* Creative and passionate about enabling people to be as independent as possible.
* Able to fulfil the travel requirements of the post.

This is a full time role at 37 hours per week. This post requires significant travel. You must have a vehicle (or transport we deem suitable) available for use as needed.
This post involves working with children and/or vulnerable adults and/or having access to significant information about them. It will be subject to a Disclosure and Barring Service check.
Interviews will be held via Teams and will take place on 4th and 5th December.
