Job summary The post holder will be a member of the Complex Physical Health team (CPH) within the Community Adult Learning Disability Team. They will provide specific skilled support to aid the CPH nursing team in the assessment and management of adults with a learning disability and Complex Physical health needs across the borough of Bexley. They will help support with training to service providers and carers across the borough of Bexley. The post holder will be expected to facilitate group work with patients and carers. They will share a caseload with the Complex Physical Health nurses and will be expected to work unsupervised at times. Main duties of the job The Specialist Support Worker will work as part of the Complex Physical Health Nursing Team with a shared caseload of people with Learning Disabilities and Complex physical health in the community who require support to maintain and improve their physical health and wellbeing.
