Job summary We are looking for a highly motivated and enthusiastic person to join an innovative wellbeing team working with women with primary care mental health and or/ substance misuse needs. The purpose of this post is to support individual's access social opportunities (e.g. hobbies, volunteering, education, support groups) by conducting assessments and providing one to one support, advice, and signposting. The social prescriber will also refer to other organisations (e.g. debt advice, counselling, social care) if needed, and provide support to access these. Experience of working with adults with mental health problems is essential, as are good written and interpersonal communication skills. Main duties of the job This social prescribing role is focused on supporting people with primary care mental health needs and within the prison, and their supporters in the community, enabling and assisting them to meet, social care and wellbeing needs, and facilitating engagement within services. The post holder will contribute to the ongoing assessment, planning delivery and review of activities and interventions against identified, social care and wellbeing needs, for an allocated group of service users. The role will also require undertaking and delivering defined activities and interventions for a wider group of identified service users across a range of geographical areas. This may be either on a one to one basis, or as part of a group activity. A fundamental part of the post is to provide a high quality, non-medical, social prescribing service to people with primary care mental health needs who may also be experiencing symptoms of mental distress e.g. mild to moderate anxiety and depression. Working from a person-cantered perspective the Social Prescriber will help people identify issues underpinning their distress to identify/signpost and support from within the community. The Social Prescriber will then facilitate contact with these supports to promote wellbeing and encourage social inclusion.
