The cardiology team at NNUH are looking for experienced cardiac nurses to join our heart failure specialist nurse team. This is an exciting opportunity for someone with significant experience of cardiac nursing wishing to join a highly professional multi-disciplinary team.
In the outpatient setting, nurse-led heart failure clinics are run to review medications and to offer education and self-management strategies. The heart failure service has a multidisciplinary approach, and as a team, we work closely to ensure the best care for our patients.

The role will include reviewing in-patients with heart failure from all specialties, delivering information and education to support patients and help them manage their condition. Part of the role will also be to develop the day case diuretic service at the Cromer site.
The post holder will facilitate early discharge from hospital and reduce readmission rates for patients with a diagnosis of heart failure. They will work collaboratively with the Heart Failure Specialist team, GPs, Community Heart Failure Nurses, and Community Matrons to meet NICE guidance in the diagnosis and treatment of heart failure and ensure that all heart failure patients and their carers have equitable access to appropriate education, advice, and support. The post holder will develop and deliver additional clinics at Cromer Hospital (with access to Consultant advice and supervision). The post holder will work with the heart failure services to develop care pathways that provide access to palliative care services.

* To provide a comprehensive nursing service for adult patients with heart failure.
* To manage a specified case load of patients, providing education, information, and support.
* To work closely with the Consultants, medical and nursing staff, multi-disciplinary team, and Heads of Department within the Directorate.
* To ensure that patients are seen within recommended time scales and targets where these are in place.
* To develop protocols and standard operating procedures (SOP) defining heart failure care pathways, including IV diuretics, admission avoidance, shortening length of stay, up-titration of medication and any other relevant clinical processes and pathways.
* To visit inpatients on the wards as appropriate to ensure continuity of care and to give advice/support.
* To accept referrals from medical/nursing staff within the Trust for newly-diagnosed and existing patients with heart failure.
* To appropriately discharge patients from their caseload who are stable within divisional guidelines.
* To respond to telephone enquiries and facilitate/co-ordinate admission or changes in appointment dates as warranted by the patient’s clinical condition.
* To administer intravenous diuretics and other medication as clinically indicated and facilitate the optimisation of medication and other treatments.
