Creative Support is a not-for-profit provider of care and support to people with a wide range of needs. We are looking to recruit Day Support Workers at our Day Centre in Wellingborough, helping adults with learning disabilities to make connections within their local communities concentrating on building social networks, developing skills, broadening relationships and increasing community inclusion. This will include providing practical, personal and emotional care and support to our Service Users. The role will include providing support in a person centred way - responding directly to the needs and wishes of the service user and supporting in 1:1s or small groups, supporting individuals when they access the local community and connecting them to their communities. You will be required to help facilitate person centred planning meetings with the individual, their families, carers and others involved in their lives and promote, develop, nurture and support the maintenance of friendships and networks and work in partnership to sustain circles of support. 32.5 hour contract is available. You must have previous experience of working within the Health and Social Care sector. It is preferable that you also have a clean driving license and are willing to drive a company minibus to help clients to access the local community. The Day Centre is located near to the town centre in Wellingborough making this easy access for bus routes and to walk to the local facilities.
