We are looking to recruit a clinical Supported discharge Matron to be part of the BHFT East Supported discharge service. The role will be based in WPH working with the acute discharge team you will be expected to carry out face to face assessment within the acute as well as work with the hospital MDTs, including physicians, dietitians, and specialist nurses, to offer a joined up, integrated service. This is a 1.0WTE Band 7 Permanent Post. You will join a small team of clinicians the team will be based in RBH

To be considered, you must have any of the following:
• Registered Nurse educated to degree level
• Community specialist practitioner / would be desirable
• Advanced clinical skills
• Experience of working in the acute/ community
• Multidisciplinary team and partnership working
• Team leadership
• Management qualification
