Job Title: Social Worker – Adult’s Hospital Discharge Team Location: Liverpool Rate: £43.78 per hour Liquid Personnel is currently recruiting for experienced Social Workers to join the Adult’s Hospital Discharge Team at Liverpool City Council. What will your responsibilities be? You will support individuals during the hospital discharge process, ensuring their transition from hospital to the community is safe, efficient, and person-centered. You will carry out assessments, develop care plans, and work collaboratively with multidisciplinary teams to provide tailored support to meet each individual’s needs.
