Job summary You will be involved in maximising the health of children, families, and the community, through health promotion, disease prevention, and other activities designed to improve wellbeing and achieve health gain. You will work closely with the Specialist Community Public Health Practitioners (SCPHN) to identify and support children to be 'ready for school' and to provide other enhanced time specific pieces of work under the supervision of the SCPHN. The Community Nursery Nurse Team has six members who work closely together across two bases in Chesterfield. They also work with the Health Visitors and the Locality Managers. The Team are hardworking and collaborate to coordinate the delivery of the preschool contacts and communicate well with the Heath Visiting Team to ensure children's needs are met and families receive the support required. Main duties of the job Support children and families in their homes and other environments by carrying out developmental assessments and planned individual programmes of care under the leadership of SCPHN. Visit families in a variety of community settings and undertake health promotion activities under the supervision of the SCPHN. Support parents and children with a range of school readiness activities. Promote health enhancing activities in partnership with other health professionals, statutory, voluntary, community organisations and members of the local population. Maintain the highest standard of confidentiality and integrity when communicating with service users and the wider public. Communicate with clients, health care professionals and the wider team including local Derbyshire County Council using effective communication skills.
