Clinical Practice Based Prescribing Pharmacist - Tameside
gtd healthcare is one of the largest NHS commissioned Primary and Urgent care providers in North West England. We have 2 exciting opportunities that have arisen for enthusiastic and dedicated Clinical Pharmacists to join our practice teams and provide care for our patients in the Tameside area.
The Clinical Prescribing Pharmacist will have General Practice experience and be keen to be part of a larger organisation as well as a smaller practice team. Our practice populations are diverse and multi-cultural with a strong community spirit. You will be part of a busy and proactive team working alongside clinical and non-clinical members of the team and the wider organisation.
The successful candidate will participate in and support the Deputy Lead Pharmacist in medicines optimisation to deliver change for the benefit of patients and/or the organisation. You will be a key member of the senior clinical team, being proactive in providing safer prescribing and offering expertise in clinical medicines advice, while addressing both public and social care needs of patients in their care.
The role is pivotal to improving the quality of care, leading innovation, and being aware of operational efficiencies. The extensive footprint of the organisation can facilitate a broad depth of skills and experience to be developed in numerous environments and with the benefits of exposure to in and out of hours working.
Main duties of the job
* Provide support to other clinicians with regards to prescription and medication queries/repeat prescriptions, demonstrating clinical reasoning and critical thinking, as well as undertaking comprehensive structured medication reviews and assessing/processing repeat prescription requests.
* Provide advice, care and medicines management for patients either via telephone, video or face to face.
* Act as a positive role model supporting junior staff while offering the ability to work flexibly between urgent care and primary care to provide safe, clinical and expert care.
* Undertake clinical medication reviews to proactively manage patients with complex polypharmacy.
* Ensure that services integrate with community and hospital pharmacy to help improve patient outcomes, ensure better access to healthcare and help manage workload.
* Lead on the development of innovation bids, new services or a new mode of service delivery to meet the demands of the changing healthcare environment.

Qualifications
* Mandatory registration with General Pharmaceutical Council.
* Specialist knowledge acquired through post-graduate diploma level or equivalent training/experience.
* Independent prescriber or working towards/intent of gaining independent prescribing qualification.
* Membership of the Royal Pharmaceutical Society (or equivalent).
* A member of or working towards Faculty membership with the Royal Pharmaceutical Society (or equivalent).
Experience
* Experience of working within primary care and/or general practice as a clinical pharmacist, with an understanding of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing.
* Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences.
* Able to obtain and analyse complex technical information and utilise data to improve clinical outcomes.
* Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to seniors or GPs when appropriate.
* Produce timely and informative reports.
* Gain acceptance for recommendations and influence/motivate/persuade the audience to comply with the recommendations/agreed course of action.
* In-depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
* Is able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long-term conditions.
* Able to undertake clinical audit and suggest changes of practice based on outcomes.
* Minimum of 2 years post-qualification experience.
