You will be joining KMPT at an exciting time. The work around the implementation of the Community Health Framework will enable Social Work to develop a strong profile within secondary care mental health service to improve patient experience and complement our existing MDT.
As a social worker, you will provide a high standard of support and care delivery concerning social care needs to those service users whose ability to cope with the tasks of living are threatened or impaired by mental health issues, emotional, psychological, and social problems.
You will be working in the West Kent Home Treatment Team, located at Priority House, Maidstone. The Team covers the West part of Kent, including Maidstone, Tunbridge Wells, and Edenbridge.
You will work closely with other agencies and individuals to support service users with a clear focus on recovery, social inclusion, and social justice. The role will entail assessment, support planning, and the opportunity to provide 1-1 strength-based therapeutic interventions.
You will act as Lead Health Care Professional (where applicable) with individuals who have serious and enduring mental health difficulties.
