Job overview
The trainee advanced clinical practitioner (ACP) endeavours to achieve through supervision, clinical and academic competence to become a qualified ACP. As a trainee ACP, they will be an integrated member of the Cardiology team aiming to develop the capabilities to autonomously deliver a Cardiology service to the Emergency portals, Inpatient bed base, and Outpatient Clinics across both UHNM sites - Stoke and County. Through physical assessment, diagnostic testing, and interpretation, formulation of management plans will be made to ensure safe practice either as an Inpatient/Outpatient or referring to other specialties.
Main duties of the job
1. Practice as an exemplary role model throughout all four pillars of advance practice.
2. Advanced knowledge, skills, and judgement for high-level and complex decision-making in an extended scope of practice.
3. Completing a full holistic health assessment using a range of assessment methods whilst working with patients, relatives, and carers.
4. Request imaging and investigations to underpin and support clinical decisions and diagnosis.
5. Make independent diagnostic decisions based on an expert clinical assessment and investigation interpretation when appropriate.
6. Make complex clinical decisions based on sound evidence base practice and clinical expertise.
7. Initiate, rationalise and suspend prescribed treatments and medication, therapies, lifestyle advice, and care.
8. Carry out invasive and non-invasive diagnostic and therapeutic procedures where appropriate.
9. Refer to other health professionals and services as appropriate.
10. Discharge patients from the service as required.
11. Maintain meticulous evidence of clinical specific competencies of advanced skills.
12. Exercise risk management using professional judgement.
13. Collaboratively work throughout all necessary multi-disciplinary members.
14. Provide clear, clinical leadership across the whole clinical area, trust and partnerships when necessary.

Detailed job description and main responsibilities
In addition, the trainee ACP has a professional responsibility to themselves and the trust to achieve academic competency and contribute to the remaining three pillars of advanced practice which are leadership and management, education, and research. The trainee ACP will develop a commitment to innovation and governance through clinical audit and research. This embeds best practice is strived for, innovation is promoted, and the need for change is constantly being challenged, paying particular attention to supporting patients care closer to home as part of admission avoidance and acute care at home.
The trainee ACP has a professional responsibility to ensure their progress, both clinically and academically, to enable them to qualify as an accredited advanced clinical practitioner with Health Education England and progress into the role of a qualified advanced clinical practitioner once all competences have been achieved.
