Summary: Manage a caseload of looked-after children and young people, providing consistent support, guidance, and planning to ensure positive outcomes and empowerment towards independence.
Responsibilities:
* Conduct regular assessments and reviews of looked-after children and young people.
* Develop and implement tailored through-care plans to ensure safety, well-being, and personal development.
* Collaborate with foster carers, family members, and professionals from various agencies to provide a holistic support network.
* Prepare reports and assessments for reviews, care planning meetings, and court proceedings.
* Offer advice, advocacy, and support to young people transitioning from care to independence.
Qualifications:
* Qualified Social Worker registered with Social Work England.
* Experience working with looked-after children, preferably in a through-care or leaving-care team.
* Strong understanding of child protection legislation, safeguarding procedures, and care planning processes.
* Excellent communication and relationship-building skills, particularly with young people.
