A very exciting new opportunity has arisen for a Benefits Realisation Lead to join us to support our ambitious plans for our Hertfordshire and West Essex ICS sharing of digital records.
The strategy for HWE ICS describes an integrated and person-centred model of care, designed to meet the needs of our population, delivered in local neighbourhoods. Improving care for people with frailty and end of life care is a key component for managing Urgent Emergency Care (UEC) demand. To deliver this strategy, the ICB have identified five key priorities for the next two years covered in the medium-term plan.
As the Benefits Realisation Lead for the Hertfordshire and West Essex (HWE) ICS you will continue to support the ICS Digital Transformation team to bring together information from the records held by the different health and social care organisations that our population receive care from. This allows health and care professionals to access a real-time summary of key information, safely and securely as they deliver care to our population. A particular focus for the post holder will be to support HWE ICS to scope, identify, baseline and plan the benefits realisation approach to implementing a digital advance care plan (ACP) solution to compliment our current technical solutions, strategies, architecture and data sharing platforms in line with the strategic digital roadmaps for our ICS partners and beyond into neighbouring ICSs, where relevant interfaces are required.

Improving care for people with frailty and end of life care is a key component for managing Urgent Emergency Care (UEC) demand. One of the ICS's agreed priority programmes is to implement digital advance care planning (also known as personalised care and support planning) as part of the identified UEC projects, will support achieving the 25% reduction in frailty non-elective admission activity.

Knowledge, Training and Experience
Essential
1. Excellent understanding and demonstrable approach to benefits management
2. Knowledge and Experience of Digital Transformation and Information exchange systems within NHS, healthcare and/or social care setting
3. Experience of substantial digital change programmes such Shared care records, interoperability and digital exchange of information
4. management methodology qualification (APMG Change Management Practitioner or equivalent certification), or similar level experience
Communication Skills

5. Must be able to provide and receive highly complex, sensitive or contentious information, negotiate with senior stakeholders on difficult and controversial issues, and present complex and sensitive information to large and influential groups
